Tobacco Dependency Advisor

The postholder will be part of an exciting new Walsall Healthcare NHS Trust clinical service delivering intense smoking cessation support to smokers using our services. Initially the focus will be on patients admitted to hospital and community inpatient settings. Underpinning the service will be the QUIT philosophy of ask theQuestion,Understand the level of addiction, provideInformation &Treatment.
To deliver this effectively, the successful applicant will need excellent communication, motivational and organisational skills and work as part of a tobacco dependency team. The Tobacco Dependency Advisors will have their own delegated caseloads, working under agreed pathways and within defined competence. Working under unsupervised guidance, they will be expected to motivate and engage sick smokers, provide individualised patient management plans whilst in hospital, facilitate follow-up support post-discharge and ensure effective handover of care to established community services.
Some weekend working (most likely Saturday mornings - to be confirmed) will be required on a rota basis.
PRINCIPAL ACCOUNTABILITIES
1. Support our patients to adopt healthy lifestyles and provide specialist smoking cessation advice
2. Provide evidenced-based smoking cessation advice across inpatient settings initially, though this will extend to other settings and patient cohorts in line with the requirements of the NHS Long-term Plan
3. Actively signpost to available smoking cessation resources in preparation for patients being discharged from inpatient settings
4. Provide staff with updates and advice to support patients to seek support to stop smoking.
5. Work collaboratively with colleagues and partner organisations to ensure smoking and tobacco related health improvement information is available.
6. Be aware of the relevant areas’ policies with reference to fire, accident reporting, infection control and health and safety.
7. Establish good relations with all grades of staff to ensure that good communication is maintained.
8. Work in a manner that always protects confidentiality.
Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 286,700, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor Hospital houses the full range of district general hospital services under one roof. The £170 million development of our Pleck Road site was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities. Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children’s Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre. This is scheduled for completion this year.
We provide high quality, friendly and effective community health services from some 60 sites including Health Centres and GP surgeries. Covering Walsall and beyond, our multidisciplinary services include rapid response in the community and home-based care, so that those with long-term conditions and the frail and elderly, can remain in their own homes to be cared for wherever appropriate.
- Support patients in adopting the correct techniques for effective NRT delivery of pharmacological treatments available to smokers, indications of their use, contraindications and cautions as applicable to each.
- Undertake and document comprehensive inpatient and community smoking assessments to include full smoking history, past quit attempts, use of nicotine replacement therapy (NRT) and current motivator(s) to quit
- Maintain up-to-date knowledge of evidenced
- based smoking cessation interventions
- Deliver a patient-led treatment plan to promptly manage nicotine addiction and prevent symptoms of withdrawal in accordance with national and local guidelines
- Knowledge of the potential side-effects of each pharmacological treatment
- Monitor effectiveness of treatment programmes set for individual patients and adjust as necessary
- Undertake administrative duties relating to referrals, team activity and clinics on the trust’s systems, including Careflow, and Pharm-outcomes and should be able to use MS Office software including Word and Excel, ensuring timely dissemination of follow-up appointments, NRT products and multidisciplinary correspondence where appropriate.
