Walsall Healthcare NHS Trust is pleased to be offering an opportunity for an experienced Occupational Therapist or Physiotherapist to be part of the Intermediate Care Team, managing a team of Therapy staff providing rehabilitation, supporting hospital discharges and hospital admission avoidance. We have a high commitment to providing high quality care that is patient centered. There is opportunity to gain experience and leadership in the various pathways in Intermediate Care including supporting hospital discharges and community rehabilitation, providing therapy intervention in order to prevent hospital admission or support patients in returning home. We are an integrated service, working with social care.

You must be a qualified Occupational Therapist or Physiotherapist with post registration experience at band 6 level. Excellent time management, the ability to work autonomously and under time constraints along with excellent communication skills and working with the wider multidisciplinary team are also essential skills for this role. Experience of leadership and management of junior staff would be beneficial.

The service requires 7 day working.

To provide a specialised therapy service in the area of Intermediate Care/Urgent Care.
To undertake a leadership role within ICS, supporting the Principals and other advanced practice with staff in planning, coordinating, delivering, evaluating, enhancing and analysing the ICS Therapy Service provided to the Trust, on a day-to-day basis.

To take a lead role in the assessment and treatment of patients, within the designated specialist area of ICS, which include those with highly complex and/or chronic presentations. To contribute towards the overall management of patients by providing treatment as indicated, and to maintain records as an autonomous practitioner.

To undertake evidence based audits to further own and team’s clinical practice. To recommend changes which may lead to implementing specific alterations to practice or contribute to amended service protocols.

To participate in clinical pathway development and implementation.

To supervise, educate and assess the performance of therapy students in aspects of ICS.

To provide advice to other professionals including those from external agencies on ICS planned and unplanned practice.

To be professional and legally responsible and accountable for all aspects of your own work including the management of patients in your care. To ensure a high standard of clinical care for the patients under the remit of the post in Community and support more junior staff and peers to do likewise.

To accept clinical responsibility for a defined caseload of patients within ICS and to organise this effectively and efficiently with regard to clinical priorities and use of time.

To interpret and analyse clinical and non-clinical facts to form accurate assessments and outcomes in a wide range of highly complex conditions within Community, and to recommend the best course of intervention, and to develop with the multi-disciplinary team comprehensive rehab plans.

To undertake a comprehensive assessment, including those with a highly complex presentation, using investigative and analytical skills, and to formulate individualised management and treatment plans, using clinical reasoning, and utilising a wide range of treatment skills and options to formulate a programme of care.

To take a lead role in the organization of group sessions.

To communicate effectively and sensitively with patients, relatives and carers to maximise rehabilitation potential, to ensure understanding of the condition and, where appropriate, to motivate and provide reassurance.

To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals. To promote
