Mrs D was frail and unwell. Her son was her main carer, but he wasn’t coping. We stepped in to help, but they were hostile to outside agencies. They’d always lived together and were terrified they’d be split up. Our social worker did a great job persuading Mrs D to get hospital treatment and a temporary stay in a care home. In an epic effort throughout COVID, she gained the son’s trust and sorted all the repairs, carers and nurses needed so that Mrs D could come home to live with her son. We also got Mr D regular respite and a personal budget awarded in recognition of him as her carer. After all, there’s nothing more important than living the life you want.

**The Role**:
As an Independent Living Assessor, you will be joining a Reablement Team which works with clients on a goal focused programme. You’ll be at the forefront of helping people develop the skills and confidence to live at home and stay independent and safe. Specifically, you’ll undertake functional assessments of people with disabilities, or those recovering from injuries or illness, then set goals and review progress. There’s lots of face-to-face contact and you’ll be visiting people at home to assess their circumstances and fit basic equipment.

You’ll be able to draw on the skills of a multidisciplinary team of community nurses, social workers, occupational therapists, GPs, mental health workers and many more to access the care and support that’s needed. You’ll also work closely with a range of external partners to tailor a programme for each individual service user. This is a critical role in meeting the current drive to free up hospital beds and you can expect to be busy. At the same time, it’s highly rewarding work where you’ll make a real difference to people’s lives.

**About You**:
Well-organised and caring with great communication skills, you’ll be committed to helping people stay independent for as long as possible.

You’re likely to have some knowledge of the conditions that can lead to disability and their impact on individuals.

You’ll also be aware of the range of services available to people with disabilities such as assistive technology and reablement programmes.

A systematic attitude, a thorough approach to record-keeping and good attention to detail are all essential. Basic Microsoft Office skills are important. You must be happy to carry and fit small items of equipment.

Plenty of training is available to help you increase your skills and knowledge.
