They’d always lived together and were terrified they’d be split up. Our social worker did a great job persuading Mrs D to get hospital treatment and a temporary stay in a care home. In an epic effort throughout COVID, she gained the son’s trust and sorted all the repairs, carers and nurses needed so that Mrs D could come home to live with her son. We also got Mr D regular respite and a personal budget awarded in recognition of him as her carer. After all, there’s nothing more important than living the life you want.

**The Role**:
As an Independent Living Assessor working for our Complex Care Services Review Team you can make your own powerful contribution to Westminster’s success. Our aim is to support adults with complex health and social care needs at home for as long as possible, and use the appropriate assessment and review tools to ensure their support plan meets their needs. Setting goals and coordinating support, you’ll work alongside your colleagues and our external partners in a way that is most beneficial to each service user.

Identifying where assistive technology will help a user become more independent and remain living in their own home, you’ll process incoming referrals and monitor current cases to ensure an appropriate service is provided. Responsible for the processing of accurate and concise records, referrals and reports, and liaising and negotiating on behalf of users with other departments and agencies, you’ll also deal with customer enquiries and provide information and advice relating to the services we provide.

You’ll have a wide range of additional tasks - these include assisting users to complete self-assessments and make informed choices about the services and treatment they need, and agreeing personal budgets based on an appraisal of the user’s finances. Committed to encouraging the independence, dignity and privacy of individuals and their carers, we’ll also expect you to enhance your professional development by attending training courses and workshops, and keep up-to-date with changes to legislation relating to disabled adults.

**About You**:
With passes in GCSE Maths and English or their equivalent, and a Level 3 Diploma/NVQ, you’ll have a good understanding of the role of care coordination and the Trusted Assessor, and of the conditions that can lead to disability. It’s important that you have a basic knowledge of the range of services available, and in addition to an understanding of the role of complex care in the community, you’ll be aware of anti-discriminatory practice and the needs of diverse communities.

Capable of working in a multi-disciplinary team and with our users’ families and support networks, you’ll be skilled at prioritising tasks and organising your time effectively. We’ll expect you to have the ability to work systematically and present clear and concise written and verbal reports, and you should also be ready to undertake the training you’ll need to fit small pieces of equipment and become proficient at approved manual handling techniques.

An enthusiastic, quick learner and good at problem solving, you’ll have well developed experience of working with physically disabled or older people, preferably in a Local Authority setting. Our aim is to support adults with complex health and social care needs at home for as long as possible. We review people face-to-face and on the phone to ensure that their support plan is sufficient and meets their needs - with this in mind you should have excellent verbal and written communication skills, and the ability to use our computer data base (Mosaic). We’ll also expect you to be capable of having difficult and sensitive conversations and possess the financial acuity to help people with their Direct Payments to employ personal assistants.
