Mrs D was frail and unwell. Her son was her main carer, but he wasn’t coping. We stepped in to help, but they were hostile to outside agencies. They’d always lived together and were terrified they’d be split up. Our social worker did a great job persuading Mrs D to get hospital treatment and a temporary stay in a care home. In an epic effort throughout COVID, she gained the son’s trust and sorted all the repairs, carers and nurses needed so that Mrs D could come home to live with her son. We also got Mr D regular respite and a personal budget awarded in recognition of him as her carer. After all, there’s nothing more important than living the life you want.
**The Role**:
As an Independent Living Assessor you can make your own powerful contribution to Westminster’s success. You’ll be a key member of our Community Independence Service (CIS), which is an integrated service designed to help adults with complex health and social care needs. We provide nursing and therapy care, supporting people with disabilities or who are recovering from injury as they develop the confidence required to remain independent at home. It will be your task to undertake our customers’ functional assessment and set goals, coordinate support and undertake reviews to ensure they’re independent and safe as they go about their day-to-day lives.
Working with external partner agencies and alongside colleagues in the wider multi-disciplinary CIS and Adult Social Care Service, you’ll be responsible for assessing, coordinating and monitoring each customer’s reablement programme and overall progress with the aim of minimising the support required. You’ll also arrange for the provision of specialist equipment and adaptions, and ensure that customers are able to carry out activities safely and with a high degree of independence.
But your work won’t stop here - we’ll expect you to process incoming referrals, monitor and review cases to ensure an appropriate service is in place, and develop support plans for customers who have ongoing care needs. You’ll also complete a discharge core assessment for each customer on conclusion of their programme, be responsible for the completion and processing of accurate records, referrals and reports, and liaise on behalf of customers with your colleagues in the department, NHS staff and with other agencies and organisations.

**About You**:
With good knowledge of the essential role of care coordination within a social care/health setting, you’ll have an understanding of the conditions that can lead to disability and their impact on people, and an awareness of the range of services available. It’s important that you’re familiar with anti-discriminatory practice and the needs of diverse groups, and your knowledge of the role of Occupational Therapy and Physiotherapy in the community will allow you to work successfully in a multi-disciplinary team and within the context of a service user’s family and support networks.
A highly motivated team worker and ready to receive supervision and advice from qualified staff and managers, you’ll be willing to undertake professional learning and development. Committed to supporting peoples’ desire to be independent and remain in their own homes while exercising control of their day-to-day life, you’ll possess the ability to assess users’ needs, develop reablement programmes, monitor progress, modify plans and recommend appropriate equipment and adaptations.
Capable of communicating clearly to users, colleagues, carers and managers, you’ll be committed to working in equal partnership with customers, and skilled at prioritising tasks and organising your time effectively. We’ll also expect you to be ready to document work systematically, present concise written and verbal reports, and develop support plans to meet our users’ ongoing needs. When it comes to qualifications, you should have passes in GCSE Maths and English or an equivalent Diploma/NVQ 3, and ideally be Care Act trained and a competent Trusted Assessor.
