We have a fabulous job opportunity for you to join our awesome team as our Support Coordinator. We support our brilliant customers, families and singles who are recovering from mental health issues and / or are offenders rehabilitating back into their community.

They live in a range of properties which are either self-contained or shared across Swindon. Most of our properties have an on-site office which our teams use. We help our customers by tailoring our support to the individual, focusing on what’s strong, not what’s wrong.

As a Support Coordinator, you’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. You’ll create bespoke and personalised support plans with and for your customers. Each plan will look to meet our customer’s individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements.

You’ll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.

**About you**

As our Support Coordinator, you’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.

To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe) You’ll be given the latest IT kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system. This means that you don’t have to be in the office as you can complete support plans on the go.

To make sure our customers are well supported, it’s important that you’ve had a job in a supported housing or housing services environment before. Don’t worry, we won’t drop you in the deep end, we’ll help to you understand how things work at Home Group

You’ll need to travel around to spend time with your customers. You could do this job using public transport, or if you drive and have access to a vehicle, that would also be amazing (we pay your expenses to cover this use)
- When you join, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one._

**Working hours**

Your hours and days are to be agreed. You’ll work between the hours of 9am and 5pm, Monday to Friday.

We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you

You’ll work alongside our fabulous team who are super passionate about supporting customers to reach their goals and ensuring they have a choice and a voice. To us were not just a team we’re a ‘work family’.

Our fantastic managers are Debbie and Becki who have between them worked at Home Group for 10 years, so are perfectly placed to support you as you join. There are 11 of us in the team and we don’t just love to support our customers, we’re there for each other too
