1.To lead on co-ordinating the daily operational requirements of discharge priorities, by centralising communications across members of the MDT to assign roles in ensuring the discharge process is time effective. 2. Central point of contact for referrals; effectively triaging the waiting list reviewing critical priorities alongside the Run
- PC tool, collaborative discussions with the nursing team for admission capacity and joining with system partners to discuss daily acute changes to priorities to ensure we are being consistent in our approach to accepting the patient with the highest need. 3.

5. To be the main link with our CHC professional contact, to advance fast track funding and secure timely onward nursing home placements or appropriate packages of care. 6. Lead co-ordination for admissions and discharges; liaising with SPOC, SPA, Priory Road Pharmacy, & MDT to ensure all elements of discharge are considered to effectively and safely discharge.

11. Communicate ACP plans, utilising the EPaacs platform. 12. Provide dynamic, inspirational, and visible leadership for clinical services ensuring the needs of patients and their families are at the heart of everything that we do.

13. To represent the Hospice and champion high-quality care at the end of life and collaborative working across the wider system working towards extending our reach across the community. 14. Support the implementation of the overall Hospice strategic vision in collaboration with SMT colleagues, Heads of Department and Trustees.

15. Develop and maintain strong and effective links with Local, Regional and National colleagues within Hospices, the wider health and care system and higher education partners. 16. Development and revision of policies which pertain to optimising effective patient flow.
