**Job summary**

Care coordinators play an important role within a practice to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

The main duties of this role are to support the PCN with the collation of information on people requiring an MDT review. The focus of which is the delivery of a comprehensive model to reflect local priorities, health inequalities or population health management risk stratification. This role will be supporting the PCN with projects such as early cancer diagnoses, learning disability checks and administrative duties relating to patient care.

They work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.

**Main duties of the job**

To work closely with practice and PCN healthcare roles, the PCC is to identify and work with a cohort of people to support their personalised care requirements, using any available decision support tools such as Patient Activation Measure (PAM), templates and software

To collate all of a patients identified care and support needs and review the options to meet these needs and bring them into a single personalised care and support plan (PCSP) in line with best practice.

**Job responsibilities**

To help people to manage their needs by answering their queries and supporting them in making appointments

To provide patients with high quality, easy to understand information to assist them in making choices about their care

To assist patients to access self-management education courses, peer support or interventions that support them in their health and well-being

To provide coordination and navigation of patients, and where appropriate their carers, across health and social care services, where appropriate working hand in hand with social prescribing link workers (SPLW)

To support in the delivery of enhanced services and other service requirements on behalf of the PCN

To contribute to public health campaigns (e.g. flu clinics) through advice or direct care

To attend and participate in the delivery of multi-disciplinary teams (MDT) within PCNs.

**Job Type**: Part-time

**Salary**: £12.25 per hour
