**Who are we looking for?**

The Care Coordinator will be responsible for the overall management and scheduling of client rotas. You will ensure care resources are used efficiently, using excellent communication skills throughout all call planning and delivery. You will have a positive attitude and lots of energy, using innovation in your problem-solving.

You will make a difference in people’s lives, one by one.

**Your main responsibilities will include**:

- The overall management and scheduling of client rotas
- Regular communication with carers and clients
- Deliver high quality care and support to clients
- Participate in the on-call rota
- Represent the company at external events
- Assist in the completion of audits and subsequent action plans to improve business processes

Please note, although our office hours are 8:30-4:30, there will be an expectation to work flexible hours.

**Qualifications and Experience**:

- A relevant qualification, for example an NVQ at level 2, such as Health & Social care (adults).
- At least 1 year experience as a Care Coordinator or Care Supervisor.
- Experience with Access People Planner and Access Care Planning would be ideal.
- Extensive experience in domiciliary care.

**Job Types**: Full-time, Permanent

**Salary**: £25,000.00-£28,000.00 per year

Schedule:

- 8 hour shift
- Overtime

- Lancashire, PR7 2SL: reliably commute or plan to relocate before starting work (required)

**Experience**:

- providing care: 2 years (required)
- Coordinator: 1 year (preferred)
