An exciting opportunity has arisen for a Fail Safer Officer for Blenheim Head & Neck unit within Specialist Surgery. This post is for 6 months to start with but may go on to a longer term and will be based at Churchill Hospital.

Job overview

To ensure that all patients receive the relevant diagnostic testing and follow up appointment in line with the recommendations of the clinical team. The post holder is required to check the records of any patient on the system who has not been marked as discharged, but that doesn’t have a follow up appointment booked. Close liaison with colleagues to ensure that appropriate appointments are booked in a timely manner.

The post holder will work very closely with the Patient Pathway Administrators, PPA Team Leaders, Cancer Team and Service Manager for the department to ensure the whole patient pathway is maintained. To ensure that the information, at patient level, is accurately recorded before being reported to Service Manager

Main duties

Maintaining a registry of patients without an appointment and keeping this organised (by clinic type, date by which each patient was due to be seen, and high risk versus low risk status), accessible, and up-to-date.

Informing the Consultant, Clinical Lead and the Service Manager of any patients whom they come across who are thought to be at significant risk of coming to harm if their appointment is delayed.

Account for all patients who have been seen in the high risk sub-specialties such as Glaucoma & Medical Retina within one week of their appointment. All requests for follow up appointments for patients attending the clinics should be immediately actioned by the Receptionists or Patient Pathway Administrator. If for any reason there is no capacity for an appointment within the appropriate timescale then the relevant staff member should flag this with the Failsafe Officer to manage in liaison with the Consultant and Service Manager.

Maintain a list of patients who have been lost to follow up and try to contact these patients to understand the reasons why they might have been lost to follow up and arrange further appointments for them as required. The Failsafe Officer will need meet weekly with the Service Manager to ensure they are kept up-to-date with this cohort of patients.

Specialist Surgery is one of the Directorates that makes up NOTSSCaN Division within OUHFT. Specialist Surgery covers Ear Nose and Throat, Plastic Surgery, Oral and Maxillofacial Surgery, Vascular Surgery and Head & Neck Unit.

To use Trust computer systems and EPR to ensure patients appointments are actioned correctly ensuring the correct RTT status

To ensure that patients who are not discharged on EPR have the correct follow up appointment booked.

Use patient notes, existing databases, EPR, and Infoflex to formulate patient pathway data on individual patients.

To act as a mentor and provide on job training for new staff as required

Failsafe all patients coming through the department to ensure their data and onward appointments are correct.

To ensure that patients discharged on the electronic system have been done so correctly so as not to miss anyone to follow up

To collect additional data on an adhoc basis as required

To plan and organise own work schedule with the use of appropriate planning aids, demonstrating good time management and ensuring anticipated difficulties in meeting deadlines are promptly reported.

To ensure that secure systems are in place for the storage of all resources, including computerised information.

To develop and maintain effective working relationships with other staff in the department and the Trust ensuring that a professional and seamless service is provided. In par
