Specific Responsibilities part 1

1. Evaluate the impact of the incidents.
2. Co-ordinate and manage the weekly SIRI forum, this will include:
- a. Identifying incidents for the weekly SIRI Forum draft agenda.
b. Present the SIRI forum draft agenda to the Director of Safety and Clinical Effectiveness so that an agenda can be formalised.
c. Manage the administration of the weekly SIRI forum meeting which will include taking minutes.
3. Attend the Patient Safety Response meetings with the Chief Medical Officer and documenting the outcomes on the clinical incident forms.
4. Enter information on the STEIS system when a SIRI is declared
5. Maintain the Divisional investigation log.
6. Maintain a close working relationship with Legal Services, Complaints, Tissue Viability team, Information Governance and Safeguarding to ensure effective communication about patient safety incidents and investigations

Specific Responsibilities part 2

1. Help the Senior Patient Safety Advisor to manage a weekly teleconference with selected corporate teams to ensure a connected approach to cases.
2. Maintain links with Health & Safety locally to ensure that relevant aspects are communicated; e.g. potential Reporting of Incidents, Diseases and Dangerous Occurrences Regulations (RIDDOR)
3. Liaise with the Assurance Team, the Clinical Commissioning Group and NHS England to ensure they are notified of any SIRIs or Never Events and that 72 hour reports submitted are as requested.
4. Provide evidence and an audit trail for the completion of Duty of Candour through Ulysses
5. Together with the Senior Patient Safety Advisor support Divisions in investigating SIRIs according to Trust policy, reviewing and approving draft investigation reports along with their action plans.
6. Monitor SIRI progress and ensure deadlines are met.
7. Maintain and update files for Divisional and Trust investigations and ensure storage of all relevant correspondence in relation to these incidents.
8. Update the Ulysses incident reporting system with incident investigation information and close Divisional and SIRI incidents.
9. Write and amend the patient safety teams local standard operating procedures
10. Provide written formatted reports for patients / relatives and our commissioners if a SIRI.
