Job Purpose The postholder will lead and implement adult social cares effective commissioning, contract management and prevention strategy approach at community level to enable high quality social services to be delivered to Norfolk people via internal teams and the external market. This postholder will lead on the Integration agenda across Health and Social Care at a strategic and local level, through leading integrated health and adult social care commissioning. Agreeing joint priorities and leading the development and implementation of these across the system utilising the resources of a range of stakeholders. They will ensure the effective coproduction of commissioning strategies and the design of delivery mechanisms/models that result in cost effective outcome driven services for the people of Norfolk.

This post will build and maintain excellent relationships with multiple organisations seeking to innovate and integrate at every opportunity with regard to system and service design. Context Integrated care systems (ICSs) have been established that are partnerships that bring together NHS organisations, local authorities and others to take collective responsibility for planning services, improving health and reducing inequalities across geographical areas. Norfolk has an increasing population of elderly residents and many of these require services that can keep them independent within their chosen communities. The authority also provides services and support to those individuals, and their families, with a learning, physical or mental health disability.

The Adult Social Care function within Norfolk County Council is responsible for the commissioning of £358 millions of care services for vulnerable people in the County. Future demographic and budget pressures within the authority means that this will be an ever-challenging task with new thinking and solutions needed. The Care market in Norfolk is being pressured by increasing demand and workforce shortages, yet it is keen to work with other stakeholders to explore solutions that can ensure it can rise to the challenge. Accountabilities Lead the joint commissioning of services across health and Social care, exploring opportunities for co-production and partnership as we innovate future service design.

Lead on the development and implementation of a Health and Social care vision for integrated commissioning design and delivery in agreed areas. Lead adult social cares commissioning, contract management and prevention strategy approach at community level Lead and manage the Better care fund, proposing and agreeing areas for investment and Support across the Health and Social Care system. Lead for the evaluation for BCF and the impact on service delivery to shape future commissioning strategies. Play a lead role in the development and implementation of new ways of working with Norfolk ICS.

Lead the development of primary-care based social care integration, alongside local community and primary care health providers and District Councils. To support the wider commissioning agenda with regards to the care market in Norfolk. Ensuring there are effective relationships with providers and supporting the market to innovate in meeting future needs. Lead the alignment of strategy, policy, funding and practice, to ensure that Norfolk has a strong demand management approach.

Responsibility for a number of adult social care and joint commissioning budgets. Lead the Council/ICB response to national and regional integration issues. Contribute to the development of National Policy and best practice through response to national debates and consultation. To be an advocate for vulnerable adults in all aspects of the work.
