Job Overview:

Lancashire Council is seeking an experienced and dedicated Team Manager to lead our Hospital Discharge Team. The successful candidate will be responsible for ensuring the smooth and efficient discharge of patients from hospital settings, facilitating their return home or transition to appropriate care facilities. This role is critical in supporting the overall health and well-being of our community by ensuring patients receive timely and coordinated care.

Key Responsibilities:

Lead and manage the Hospital Discharge Team, ensuring high standards of practice and service delivery.
Coordinate and oversee the discharge planning process, working closely with hospitals, community services, and other relevant agencies.
Ensure compliance with relevant legislation, policies, and procedures, including the Care Act 2014 and the Mental Capacity Act 2005.
Conduct regular team meetings and supervision sessions to support staff development and address any issues or concerns.
Liaise with healthcare professionals, social workers, and external agencies to facilitate effective communication and collaboration.
Monitor and evaluate team performance, implementing improvements as necessary to achieve service objectives.
Manage resources effectively, ensuring the team operates within budgetary constraints.
Handle complex cases and provide guidance and support to team members on challenging issues.
Ensure accurate and timely record-keeping and documentation in line with organisational requirements.
Promote a person-centred approach, ensuring that the needs and preferences of patients are at the forefront of all discharge planning activities.
Respond to complaints and incidents, ensuring appropriate action is taken to resolve issues and improve service quality.
Participate in training and professional development opportunities to stay updated with best practices and emerging trends in hospital discharge and social care.Requirements:

Qualified Social Worker with current registration with Social Work England.
Significant experience in a social work management role, preferably within a hospital discharge or related setting.
Strong knowledge of relevant legislation, policies, and procedures.
Excellent leadership and team management skills.
Effective communication and interpersonal skills.
Ability to work under pressure and manage multiple priorities.
Strong problem-solving and decision-making abilities.
