In your role you will work closely with Occupational Therapists, Assistant practitioners, support staff, clinical assessors ( UCR team), district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes. Part of your role will include working in the Transfer of Care HUB to determine appropriate community pathways for patients being discharged from bed based services.
This post is eligible for a Golden Hello payment of 2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays .
You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care. You will be visiting patients within their own homes using a goal-orientated focus, to maximise patient's independence, safety and quality of life.
The post will involve cross site working and community visits. Part of your role will include working in the Transfer of Care HUB to determine appropriate community pathways for patients being discharged from bed based services.
A full UK driving licence and vehicle for business use is essential for this post however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. You will have a degree/diploma in Physiotherapy and be HCPC registered. You should possess excellent interpersonal and time management skills, be a good communicator and able to prioritise and work well as a member of a team. An induction period and supernumerary support will be given. You will have significant experience of working in the NHS in the UK as a Band 5 Physiotherapist and being part NHS healthcare services. - Relevant courses in Health Care of the Elderly/Falls/Orthopaedics/Palliative Care/MSK
- Proven post qualification experience with some at a more senior level
