DescriptionSuffolk County CouncilAddress: Ipswich Hospital Heath Road Ipswich IP4 5PD - HybridSalary: 34434 per annum (pro rata if part time)Hours: 10 hours per week flexible working options availableContract: PermanentA great opportunity has arisen to join the Digital Care Team as aSenior Independence and Wellbeing Practitioner Digital Discharge Coordinatorworking atIpswich Hospital within Adult Social Care.Your role and responsibilitiesYou will work as part of the Digital Care Team to support patients families and staff to use the Cassius Service particularly as part of the discharge closely with social workers and other Independence and Wellbeing Practitioners and staff from across the locality to get the best outcomes for people as well as health colleagues both within the hospital and community settings.This role will give you great hands-on experience of care technology in practice and delivering change. This work is rewarding and fast-paced.Specific tasks includeSupporting other professionals to consider care technology as part of discharge planningSupport care technology assessmentSupporting community referrals to prevent hospital admissions linking in with INT Coordinators.Managing the referral process (including benefits tracking and recording)Supporting communication and awareness raising of CassiusManaging decisions about eligibilitySupport the Alcove Team in prioritising installationsDelivering training to therapy teams both hospital and community basedShape and develop the role in line with business needs and changesWorking as part of an integrated team with NHS Social Care and CassiusWhat you will needExcellent communication skills.The enthusiasm to contribute to a multi-agency approach for responding within urgent timescales.Resilience under pressure and the ability to remain positive when challenged.Ability to adapt to changes within the role as and when required.Excellent knowledge of care technology and Cassius and how this can positively improve quality of life and maintaining adults independence.You can view a full list of requirements in the Job and Person Profile (Word) (JPP).If you believe you can succeed in this role apply.
