**Your role and responsibilities**

As a key member of our integrated health and social care network, you'll conduct asset-based assessments, provide personalised support, and deliver innovative solutions to maximise independence and wellbeing for our customers. Utilise your expertise in functional assessments, reablement techniques, and digital care solutions to create tailored support plans. If you're passionate about making a positive difference in the lives of adults in Suffolk by promoting independence, and working collaboratively in a dynamic environment, we want to hear from you.

**You will need**
- excellent communication skills to convey information clearly and effectively to clients, their support networks, and fellow professionals across various disciplines
- a passion to contribute to the team systems for responding to urgent work
- to be resilient under pressure and the ability to remain positive when challenged

**The team**
- Home First provide a short term reablement care and support service to people living in their own homes
- We will give you excellent support and supervision.
