Your role and responsibilities

We offer support and a chance to work in an interesting environment as part of a busy integrated team, with other health professionals.

You will play a key part in promoting personalisation, independence, wellbeing and choice by providing a social care service for people. You will be expected to exercise a high level of autonomous professional judgement and risk evaluation, taking responsibility for the safety and well-being of people accessing adult social care services within a health environment.

The role will include completing strength-based assessments/reviews, short term reablement plans, care and support plans and safeguarding plans in line with Supporting Lives, Connecting Communities and using a Signs of Safety and Solution Focused approach.

You will need:
  • a strong working knowledge of relevant legislation, including The Care Act 2014 and The Mental Capacity Act 2005, and how these apply in practice
  • a solid understanding of Adult Social Care Services, including safeguarding procedures and the assessment, support planning, and care management process
  • proven experience in applying a person-centred, enabling approach to support individuals in maximising their independence and wellbeing
  • confidence in managing risk and safeguarding concerns, including the development of safety plans to reduce the risk of ongoing abuse
  • experience of working collaboratively within integrated health and social care teams, including Social Workers, Occupational Therapists, Nurses, and other professionals.
