Join us and turn passion for progress into your action.Your role and responsibilitiesIn this role youll work directly with people receiving services to review their care and support plans ensuring they meet individual needs and promote independence and wellbeing. Youll be a key part of our mission to deliver high-quality personalised outcomes that are economically sustainable.On a typical day you might:assess eligibility under the Care Act 2014arrange care packages tailored to individual needsevaluate decision-making capacity under the Mental Capacity Act 2005apply safeguarding policies and proceduresreview existing services to ensure quality and compliancecomplete checklists under the Continuing Health Care Frameworkconduct phone assessments home visits and attend professional meetings.This role involves lone working in the community and youll be supported through a comprehensive induction shadowing opportunities and ongoing training.What you will needWe hope youll bring:a warm approachable mannerstrong communication skillsconfidence in a busy target-focused environmenta commitment to learning and developmentYoull work as part of a dynamic social work team collaborating with colleagues in contracts and service development and building strong partnerships across health and social care.
