Assessment Nurse

We are collaborating with a private client who are seeking an Assessment Nurse to support with assessments and plans to determine suitability of a service user for admission, across all of their sites: Newham, Croydon, Maidstone, Hampshire.

Ideally 3 assessments per week

You will be required to travel across all of their sites in Newham, Croydon, Maidstone, Hampshire.

£30 - £40 p/h depending on experience

37.5 hours a week

Locum Contract

Key responsibilities:

In conjunction with the business development team (BDT), manage referrals for complex packages of care by assessing and planning care for service users referred to to service.

To analyse and interpret clinical information and medical evidence and produce an assessment report and proposed care plan in a professional and concise manner.

Liaise with registered managers or attend MDT/staff meetings, to present assessment outcomes and clinical information to inform decisions around suitability of proposed placements.

To manage the expectations of service users/families and other health professionals, who may have an unrealistic idea of timescales.

Requirements:

Registered Nurse - RMN or RNLD, with a valid NMC Pin (would consider RGN if able to evidence relevant experience in mental health and learning disabilities)

Experience of undertaking clinical audits

Experience of undertaking assessments of complex needs

Experience of case management

Ability to cope with complex organisational issues and able to relate to a wide spectrum of professionals
