Sanctuary Personnel is currently looking for an experienced Band 6 Community Psychiatric Nurse (CPN) to take on a new ongoing contract role within the Mental Health Liaison Team.
Location: Harlow
Pay Rate: £27.17per hour (assignment rate)
Contract Type: Locum, 6-month contract/ongoing block booking.
Working hours: Monday – Friday, 37.5 hours per week (hours flexible between 8am and 8pm). Part time will be considered.
Main Duties:
Service User Care: Provide and oversee the management and care of service users, ensuring their needs are met comprehensively.
Documentation: Produce and manage accurate and timely documentation/records for all service users, adhering to your Professional Code of Conduct and Trust policy.
Assessment and Observation: Conduct and supervise assessments and observations, ensuring timely and appropriate feedback to the team.
Risk Assessments: Conduct risk assessments for individuals and identify potential hazards in the service user's environment.
Meetings and Conferences: Attend multi-disciplinary meetings, professional meetings, case conferences, ward reviews, and handovers.
Incident Reporting: Report and document incidents as required, following organizational protocols.
Mental Health Needs Assessment: Conduct mental health nursing needs assessments and initiate specialist assessments for NHS-funded continuing healthcare, in line with government policy and eligibility criteria. Present cases to the Continuing Health Care Panel when required.
Care Coordination: Collaborate with professional colleagues to proactively manage and negotiate complex transfers of care, such as social care packages and appropriate placements.
Care Planning and Review: Provide comprehensive assessment, care planning, treatment, and review of care in accordance with Mental Health care clusters. Formulate, deliver, and review individualized evidence-based care/care packages according to NICE guidance, including non-medication strategies.
Service Development: Lead in the continuing development of a high-quality service. Contribute to local emergency plans, team risk assessments, and other initiatives underpinned by evidence-based practice, care pathways/packages, and NICE and DH Guidance.
Requirements of the Community Psychiatric Nurse:
Degree or equivalent in Mental Health Nursing
Experience working as a Mental Health Nurse
NMC Registration
