Penwith PCN

1 x post hours / 5 days per week

Fixed Term Contract until 31st March 2024 with potential to continue

Base will be Morrab GP Surgery in Penzance for mileage purposes

Salary £27,288.79 per annum


As a Social Prescribing Link Worker (SPLW) you will deliver 1:1 personalised support to clients who are referred to you by the surgery team and local agencies.

You will empower clients to take control of their health and wellbeing by giving them time to focus on 'what matters to me'.

You will build trusting relationships with clients, create personalised care and support plans, and connect them to community groups and other appropriate services.

This is a community outreach role.

You will meet with clients in the community in the Penwith area and will work from Morrab GP Surgery and your home address on an ad-hoc basis.


Do you:

  • Have excellent listening, verbal and written skills, and understand the skills and techniques required for effective guided conversations
  • Have experience of working positively with people facing complex social and emotional challenges
  • Have a full driving licence with access to and use of a vehicle for work purposes

Could you:

  • Support, coach and engage with people to improve their life outcomes
  • Deal professionally, confidentially and empathetically, in an unbiased way, with people in different circumstances
  • Commit to supporting local communities to care for each other
  • Source information on a wide range of issues, using face to face and telephone conversations, literature and IT research
  • Prioritise work under pressure, whilst working with a high caseload and working to tight deadlines
Previous Social Prescribing experience, or similar, would be advantageous
