About the Role

We are seeking a highly skilled and compassionate individual to join our team as a Support Coordinator. As a key member of our Care Team, you will play a vital role in supporting our customers with complex needs to develop their skills and live more independently in the community.

Key Responsibilities
  • Develop and implement personalized support plans in collaboration with customers, their families, and other stakeholders.
  • Coordinate and lead a small team of Support Workers to deliver high-quality care and support services.
  • Conduct thorough assessments and risk evaluations to identify customer needs and develop effective support strategies.
  • Provide emotional support and guidance to customers, promoting their independence and well-being.
  • Collaborate with other professionals, such as healthcare providers and social workers, to ensure seamless care and support.
Requirements
  • Proven experience in care and support work, preferably in a leadership or coordination role.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with customers, colleagues, and external partners.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and locations.
  • Enhanced DBS check and ability to maintain confidentiality and adhere to data protection policies.
