Job Summary

We are seeking a highly motivated and compassionate Support Worker to join our team in Penzance. As a Support Worker, you will play a vital role in supporting vulnerable individuals to live independently and achieve their goals.

Key Responsibilities

  • Provide person-centered support to individuals, promoting their independence and well-being
  • Work collaboratively with colleagues and external agencies to ensure seamless support services
  • Develop and maintain relationships with service users, their families, and other stakeholders
  • Contribute to the development and implementation of individual support plans
  • Participate in on-call rosters and provide out-of-hours support as required

Requirements

  • Experience of working in a support role, preferably in a housing or social care setting
  • Excellent communication and interpersonal skills
  • Ability to work flexibly and respond to changing situations
  • Knowledge of relevant legislation and policies, including the Care Act and Mental Health Act
  • Ability to maintain confidentiality and handle sensitive information
