We have a fantastic opportunity for a Band 6 Triage Clinician to join our new Virtual Ward team based in our Care Coordination Centre at Peasedown St John.

This is an exciting collaboration working across organisational boundaries to bring together the expertise of acute services with community services.


HCRG Care Group are leading the delivery of a new Virtual Ward team and are looking to offer a fantastic opportunity to be part of a new venture, developing innovative care pathways and using remote technology to provide acute care at home and prevent avoidable admission to hospital.


As a Triage Clinician you will support our first point of contact for new referrals requiring clinical triage with a view to preventing admission and ensure a timely and responsive care pathway is decided with the patient and the point of contact.

This service runs 8am to 8pm, 7 days a week.

Main Responsibilities

As a Triage Clinician you will be:

  • Delivering evidence based remote triage skills to ensure the appropriate management of patient's needs.
  • Using skills of remote assessment, diagnosis, interpretation of tests, forward planning of care & treatment.
  • Timely recognition of deteriorating patients, appropriate response, escalation & documentation.
  • Participate in, initiate, and deliver audit & research to identify current practice
  • Delivering training & education to support up to date evidenced based practice.
  • Identifying poor practice & supporting improvement opportunities as part of a Just Culture based on reflection, supporting an evolving model.

  • NMC or HCPC registration
  • Experience of triage
  • Knowledge of national rules for NHS data capture (e.g. Referral to Treatment).
  • Remote clinical decision making education
  • Range of clinical experience in Urgent Care and Community.
  • Clean driving license and access to a car
