- Enthusiastic, self-motivated, and well-organized individual.
- Main base at Neath Port Talbot hospital with visits to GP practices as needed.
Main Responsibilities:
This role offers the opportunity to specialize in CKD. You will work under a renal pharmacist's guidance, conducting medication reviews, evaluating patient blood pressure, interpreting blood test results, promoting healthy lifestyles, and more. Also, you will participate in audits and SOP development.

Qualifications:
Essential:
- BTEC Pharmaceutical Science or equivalent (NVQ Level III Pharmaceutical Services)
- Registered with professional body - GPhC (RegPharmTech)
Desirable:
- Additional management or clinical qualification
Experience:
Essential:- Minimum 5 years post-qualification experience
- Prior experience in a relevant primary care role
Desirable:
- Skills in drug information and medication review
