Linkage Community Trust is looking for a Rapid Response Worker to join our team as we develop a new and exhilarating service in and around the Gainsborough area.


The Rapid Response post is initially for a 12 month period, thereafter a comparable position within Linkage Community Trust shall be available.

Who are You?
- You are an experienced professional working in a Learning disability setting with good knowledge of conducting assessments, designing care plans, and identifying risks to ensure that individuals independence is promoted and managed positively -
as an existing Senior or Deputy manager this is a thrilling opportunity to expand on existing knowledge while acquiring new skills

About the role:

  • You will work as part of the rapid response team across the Gainsborough area, offering short-term care and support to individuals with a Learning disability and/or Autism
  • You will assist in discharges from hospital to residential settings or the individuals' own home, and/or prevent in-patient admissions where feasible
  • You will contribute to and conduct initial assessments for individuals ensuring that person-centered support plans are implemented and any identified risks are managed positively to promote independence
  • You will aid the service in providing a quick response to service requests and act as the coordinator between the person needing support and other organizations and agencies as required
  • You will need to be flexible to requests for assessments and support, engaging at times that may fall outside of core hours
  • You should have experience working with service commissioners, responding promptly to referrals, and adapting flexibly to meet the needs of individuals accessing the service
  • You must be confident in maintaining accurate and detailed electronic records and possess good IT skills
