An exciting opportunity has arisen to join the Haematology Nurse specialist team here at WWL.

Please note there are a 37.5 hours post and 25 hours post available, please state your preference in the supporting information section.

The post holder will provide a specialist nursing care and support to patients with haematological disorders.

The post-holder will partake in nurse led clinics both outpatient and telephone clinics and provide treatment through means of non-medical prescribing.

The post holder will also be a core member of the GMCCN Northwest Sector Haemato-oncology specialist multi- disciplinary team and lead on patient and communications issues within it.

The post holder will be named the local key worker in conjunction with the other Clinical Haematology nurse specialist in the Haematology Oncology MDT with the responsibility to coordinate the patient's pathway. They will also operate autonomously at an advanced level of clinical practice providing research based expert care and advice to all colleagues and managing a caseload effectively, making the most of available time.

Main duties of the job

The main focus of this post is to provide specialist nursing care and support to the patients with a Clinical Haematological disorder. (Primarily cancer and pre malignant disorders).

The post holder will promote and deliver the highest standard of nursing care, ensuring all fundamentals of care are addressed and current, relevant policies and procedures are adhered to and regularly updated.

Detailed job description and main responsibilities

The post holder will partake in Nurse led Clinics (Outpatients) and telephone clinic specific to the speciality (and provide appropriate treatment through means of non-medial prescribing) The post holder will be a core member of the GMCCN Northwest Sector Haemato-oncology specialist multi- disciplinary team and lead on patient and communications issues within it. Deliver specialist training and education programmes to staff and patients. The post holder will provide professional leadership and expertise in the speciality of malignant and non-malignant haematological treatment. Additionally, the post holder will develop standards policies and guidelines for Haematology Oncology MDT. Additionally assist in evaluating, updating, initiating changes to policies and procedure relating to the Haematology Oncology at Trust level, Network level and nationally. Ensure all members of staff are aware of their responsibility for Child Protection issues in their area.

Planning and Organisational Duties

The post holder will participate in, and influence, care delivery by:

Operate autonomously at an advanced level of clinical practice providing research based expert care and advice to all colleagues and managing a caseload effectively, making the most of available time. Promote and deliver the highest standard of nursing care, ensuring all fundamentals of care are addressed and current, relevant policies and procedures are adhered to and regularly updated. With other Clinical Haematology Nurse specialist lead outpatient and carer communication issues and coordination of the patient pathway for patient referred to them – acting as the key worker or responsible for nominating the key worker for the patients dealing with the team. Utilise specialist skills underpinned by evidence-based practice, to assess patients physical, psychological and spiritual needs, create care plans, implement and evaluate nursing care involving patients and carers as appropriate Accept referrals from other Health Care Providers and where necessary refer to other Health Care Professionals Ensure the patients are seen within the recommended time scale and targets where they are in place. Write reports, record observations and maintain care plans Contribute to the multidisciplinary discussion and patient assessment / Care planning decision of the team at their regular meetings. Assist in the organisation and running of nurse led clinics Outpatients and telephone clinic specific to the speciality. Independently assess and monitor patients with Haematological disorders including: Physical Examination, Assessment / Investigation of symptoms, Evaluation of test results, Recommendation of further diagnostic tests Provide expert nursing advise to other health care professionals in the nurses specialist areas of practice Coordinate admissions or changes in appointment dates as warranted by the patients condition Supply and or administer medication according to Patient Group Directions or by Independent/ Supplementary Prescribing if a registered Nurse Prescriber in accordance with the Trusts Policy for prescribing and the management and administration of medicines. Provide palliative care to appropriate speciality and liaise with Palliative Care Team who have specialist palliative care need outside of the competence of the post holder. Act as a resource to primary care givers to meet the patients palliative care needs. Be fully conversant with the Trusts Haematology Protocols. Obtain informed consent for specific procedures in line with relevant Trust policies and protocol ensuring full explanation is given. Maintain relevant information clinical and pharmaceutical knowledge, critically appraise and apply information to practice

Communications and Key Working Relationships

All staff are accountable for their own professional actions undertaking all duties having regard to the NMC "Code of Professional Conduct" for nurses, midwife and health visitors, and their professional contributions to clinical governance. Works as autonomous practitioner, leading in the development of policies, procedures and guidelines, in own sphere of responsibility. Is guided by Trust protocols and codes of conduct

Responsibility for Finance

Ensure the efficient and effective use of resources in providing and delivering the service Liaise with the budget holder regarding stock control, monitoring and maintaining equipment e.g. patient information leaflets.

Responsibility for Human Resources

Ensure compliance with policies, procedures and clinical guidelines for self and others and that these reflect national standards Contribute to the development and management of the service Develop policies and procedures for the management of patients with no haematological malignancy in conjunction with other members of the MDT Take an active part in Peer Review including monitoring and measuring performance against national standards of care such as IOG and NICE. Plan, develop, formulate and review nurse led protocol and patient group directions specific to the clinical haematology service and update as in required in liaison with others. Act as a role model, inspiring staff and students with their visions and value Contribute to the implication of key performance indicators for Improving Working Lives Adhere to Trust policies and maintenance of the professional Code of Conduct.

Responsibility for Health & Safety

Compliance with the Health & Safety at Work Act 1974 – the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.

Act as the patient's advocate during treatment choices. Keep patients and carers informed, maintaining patient confidentially Act as a point of contact for patients, their families and carers, by attending outpatient clinics and inpatient wards where necessary, and by other means of communication e.g. telephone/ email. Be aware of all Clinical Haematological patients admitted as inpatients and liaise with nursing and junior medical staff involved in their care Inform appropriate patients about the impact of treatment on fertility, providing advice about contraception and, where appropriate make referrals to the Regional Fertility Centre at St Mary's Hospital. Respond to telephone enquiries and assess patients' via telephone communication Recognise patients changing needs throughout their journey and liaise with appropriate members of the multidisciplinary team for referral to the relevant services.
