Patient Flow Administrative Support

At East Lancashire Hospitals NHS Trust our vision is to deliver a first class service to the local and wider community through the development of a multi-skilled nursing workforce which currently boasts Nurse Consultants, Advanced Nurse Practitioners, Emergency Nurse Practitioners, Nurse Practitioners and Practice Education Facilitators.


We are looking to recruit a dynamic and forward thinking administrator to support delivery of our patient flow and site management service across all our 5 clinical sites.

This is a daytime part time post covering the hours of over a 7 day rota, so there is weekend rotation involved.

Only those applicants who demonstrate clearly how they meet our person specification will be shortlisted for interview.

MAIN DUTIES

In conjunction with the Patient Flow Team, develop and implement administrative processes to ensure that the patient's journey is managed smoothly and in accordance with agreed timescales including working towards the 4 hour A&E target.

Maintain an accurate and up to date filing system, ensuring safe recording of Patient Flow documentation and maintaining confidentiality.

Contact key areas such as community hospitals, paediatrics, NICU and assessment units for bed updates and information, several times a day, and ensure accurate recording of this information.

Close liaison with the Discharge Lounge, relaying information on discharges as directed by the Patient Flow Team, to ensure timely transfers of patients to discharge lounge, to assist with creating bed capacity on wards.

Assist the Patient Flow Team and the wards in ensuring that information on electronic bed boards is accurate and up to date.

Assist with liaising with surrounding hospital trusts to ensure accuracy of patient repatriation records.

Deal with requests, concerns and queries promptly from patients and members of the Patient Flow Team and MDT and escalate issues to the appropriate team member or team leader.

Record and monitor telephone queries on the appropriate documentation and escalate issues as required.
Undertake general administrative duties which facilitate the smooth running of the service.

Please Note:
new entrants to the NHS will normally commence on the first pay point of the relevant band.

COMMUNICATION
Deal with telephone calls, disseminate information and ensure that the appropriate
action is taken, whilst maintaining the Trust's policies and procedures.
Monitor bleeps for Patient Flow team during meetings as required and record and relay information and messages as required.

Act as the point of contact for the team, dealing and responding effectively with complex queries and passing on relevant information to appropriately team members.

Establish and maintain effective communication with the Patient Flow Team and members of the MDT.

TRAINING AND DEVELOPMENT
Assist in the induction and training of staff
Attend appropriate mandatory training and take part in Continuing Professional Development.
Participate in the Personal Development Review process and undertake any training/education to meet development needs.

ORGANISATIONAL RESPONSIBILITIES:

Maintain appropriate filing systems. Accurate record keeping, archiving, production and upkeep of databases.
Ordering of general office stock as directed by Patient Flow Team Leader.
Organise and prioritise workload to ensure duties are completed on time for Patient Flow meetings several times a day.

PROFESSIONAL RESPONSIBILITIES:

Be able to deal with the workload in a detached and professional manner.
Maintain confidentiality in all aspects of duty including patient information and staff personal data.
Assist in maintaining accurate records and clear documentation.

Maintain accurate and effective databases and spreadsheets, recording essential information and inputting statistical data in accordance with guidelines and policy using Microsoft Office, Word and Excel.
