Job summary

Clinical Training Fellowship

This 24-month program offers training and development under the supervision of a leading consultant and an educational supervisor. Successful applicants will have regular supervised clinical sessions in the chosen anaesthetic subspecialty.

The first 1-2 months will include an acclimatisation period and introduce fellows to working in the NHS; it will provide an introduction to anaesthesia services in the UK during which the fellow will be expected to achieve the competencies that allow them to then participate on the on-call rota. During the remainder of the first year the post-holder will rotate through different specialty theatres and can be expected to become competent in the performance, trouble shooting and innovation in general anaesthesia and their chosen specialist area. Upon completing 18 months the post holder will be expected to achieve competencies as expected for a senior trainee matching the Royal college of anaesthetist curriculum for advanced training (Appendix 1). Participation and leading clinical audit will be encouraged and when opportunities arise, they will be sign posted towards appropriate research.

Main duties of the job

An evaluation is undertaken every three months to track progress based upon which with increasing levels of responsibilities will be assigned both for on calls and routine operating theatre sessions. As well as gaining clinical skills, the fellowship scheme will provide the opportunity to develop a range of transferable skills such as:

Leadership Management Developing guidelines Conducting audit and research Understanding clinical governance

All fellows will be required to enroll themselves as trainee doctors with Royal college of Anaesthetist. Registering to the RCoA on line training portfolio will be essential for recording of clinical and non-clinical skill competencies which is structured to reflect the RCoA training curriculum.

The Directorate of Theatres, Anaesthesia and Critical Care includes preoperative assessment and pain medicine. We provide a wide range of anaesthetic services for general surgery, vascular, urology, colorectal, breast, ENT, maxillofacial, eyes, plastics, orthopaedics, obstetrics, and gynaecology amounting to approximately 20,000 operations were performed every year. The main operating suite is comprised of six operating theatres. Trauma and orthopaedics surgery are performed in a separate renovated complex of four laminar theatres and a day care ward.

Training Aims

To develop the knowledge, skills and attitudes and workplace training objectives in regional anaesthesia equivalent to a higher/ advanced level trainee in anaesthesia.

Training Objectives

To acquire an in-depth knowledge and understanding of the management of a wide variety ofanaesthesia techniques and its applicability across different specialties and in the management ofcomplex patients.

To acquire specialist skills in the regional anaesthesia including: Single shot ultrasound guided blocks; Continuous catheter techniques and their safe management in a wide range of locations,indications, regimes, kits; Advanced postoperative pain management.

Vascular anaesthesia . Open and endovascular repair of aneurysms , Management of acutely ill acute limb recircularization and specialist vascular MDT.

General and high-risk obstetric anaesthesia.

Airway endo-surgery with advanced management of difficult airway surgery.

Commencing quality improvement projects and also local audit projects.

Development of clinical guidelines for the safe performance of anaesthesia

Acquire knowledge to pass the |FRCA exam.

Involved in teaching & training other juniors and also supporting consultant colleagues.

Participate as faculty in local and national courses.

To acquire management, communication and team working skills to allow them to work flexibly and effectively both in and outside the theatres with a multidisciplinary pain team of varying makeup.

To develop communication skills with, patients, relatives and members of the multi-disciplinary team& to understand the concept and legality of consent relevant to regional anaesthesia.

Supervision

The overall responsibility for the trainee will rest with the College Tutors & the Fellowship and Programme directors.

Assessment

Assessments will be carried out at 0, 3 and 6 months and will contribute to summative assessment in thetrainees annual appraisal. 360 degree appraisal techniques will also be used.

Worked Based Assessment

The tools that are in place for UK anaesthesia trainee assessments (DOPS, CEX and CPD) will be used where appropriate. In addition the fellow will be expected to complete syllabus-based workplace assessment forms and present monthly case reports to the supervising consultant. They will be expected to contribute to weekly teaching sessions and a monthly journal club.

Fellows are required to keep a logbook following the requirements laid down by the Faculty of the Royal College of Anaesthetists.

Audit and Research

All International Fellow after the initial acclimatisation period will be encouraged to register audit and quality improvement projects with audit leads . Fellows are also encouraged to discuss potential research projects with the R&D within NGH.

Teaching

There are weekly protected teaching sessions, journal club audits are built into the timetable. The fellows are expected to run tea table -training for other juniors and consultants.

The post holder will be supported in endeavoring in any of the relevant UK examinations they are eligible for. Study leave conforms to terms/conditions of service for Trust doctors.
