Job summary

An exciting opportunity has arisen to recruit an enthusiastic and motivated Band 6 Occupational Therapist to join our innovative Care Home Intensive Support Team (CHIST).

The role is community-based with remote working, requiring the post holder to meet travel demands of the role by working across the county in care homes registered with Staffordshire County Council.

Main duties of the job

CHIST provides Nursing and Occupational Therapy staff for targeted, time limited support to specified care homes as prioritised through local system escalation processes.

You will have excellent interpersonal and communication skills and an ability to quickly develop effective relationships with care home staff.

You will have specialist clinical skills in a wide range of areas to enhance the health and wellbeing of care home residents providing hands on clinical support, advice and guidance based on resident's needs. This could include support and advice with; moving and handling, falls management, promoting meaningful activity, personalised care planning, nutritional advice/signposting, and navigating links with other Health and Social Care Services.

The successful applicant will undertake tasks concerning the day-to-day organisation and operation of the team, working with the team leaders as required.

You will be skilled in risk management and demonstrate clinical leadership skills with an ability to prioritise the support needed in a care home, and escalate any issues appropriately.

You will be required to promote effective team working in care homes and have a willingness to share your experiences and knowledge with others, in accordance with evidence based practice.

Job responsibilities

1. To assess residents care/support needs when required, providing advice and support whilst working closely with the care home staff to formulate an appropriate plan of care for residents.

Intervention may be required to support with:

Infection Prevention and Control

Maintaining safety and highlighting areas of concern

Moving and handling

Falls management strategies

Increased functional ability

Improvement in health and wellbeing including mental health

Interventions in disease/condition management

Prevention and reduction of health inequalities

Acute hospital admission avoidance

Interventions around specific long term conditions and clinical need

Palliative and end of life

Provider failure

1 To maintain and develop skills and knowledge to deliver a range of interventions (including physical interventions, training and support to care home staff, onward referrals for service users) and sets an example of good practice to other team members including skill sharing.

2 To be involved in projects to support the Local Health Economy.

3 To maintain sensitivity at all times to the emotional needs of the service user and their carers where there may be communication difficulties and barriers to understanding offering both reassurance and empathy.

4 Displays exemplary personal and professional standards of behaviour and integrity at all times.

Qualifications & Training

Essential

Degree in occupational therapy or equivalent Registration with HCPC Mentorship qualification CPD Folder / Evidence of ongoing learning/ development Post qualification courses in occupational therapy Attendance at relevant networks Special Interest Groups Willingness to take students and complete student education training Demonstrate understanding of Equality and Anti-discriminatory practice

Desirable

Modules at Masters level Post Graduate studies in relevant occupational therapy field such as; rehabilitation, end of life care, long term conditions/palliative care or related/relevant subject Evidence of further clinical study Up to date field work/pre-registration student training.

Experience

Essential

Relevant experience to the role including knowledge and management of long term conditions Ability to manage time and workload effectively Working in a multidisciplinary team To have knowledge & experience working with older adults

Desirable

To be able to negotiate and manage conflict in clinical situations To be able support in service improvement To have knowledge & experience working in both nursing & residential care settings Experience of working in Older Adults Mental Health Services
