You will be part of the Community Rehabilitation Service providing high quality rehabilitation services to patients with a range of physical health conditions. This includes Stroke, Parkinsons Disease, Multiple Sclerosis, Spinal Injuries, Cancer, Post Surgery, Amputees, General Frailty, plus a wide range of other conditions. Treatments are carried out in an outpatient setting, as well as clients homes and nursing homes. You will provide clinical and administrative/ triage support to the team.

The post holder will work jointly with qualified therapists with complex patients, and will work autonomously within their scope of practice. Treatment sessions aim to maximise a patients level of function and independence, and minimise risk. You will require excellent communication skills, to be able to communicate with patients with complex communication needs, to be able to communicate with the Multidisciplinary Team, and wider NHS teams and external agencies such as care agencies. The role involves managing a caseload of adults in close liaison with the team, many of these patients will have multiple complex health needs.

Within scope of practice this role provides the opportunity for the post holder to undertake independent, holistic assessments with service users in their normal place of residence using manual assessment techniques, analytical and clinical reasoning skills. The role involves daily documentation in patient notes (paper and/or electronic) the regular recording and analysing of outcome measure data and the completion of discharge summaries. The Assistant practitioner role supports the work of a range of different professions ensuring that the individuals physical, psychological and emotional needs and preferences are met. The role will involve delivering high quality, flexible and personalised Physiotherapy interventions, and will involve joint working with Occupational Therapists and Neurological Nurses.

Whilst working independently the post holder will be accountable to the appropriate registered health professional and adhere at all times to defined pathways. This is a dynamic post that will be evaluated and adjusted to reflect the needs of the service, local population and feedback from users and staff, including post holders themselves. You will be expected to actively participate in the development of the post to ensure it meets the needs of individuals receiving the service and their carers.
