As one of the main mental health providers within Greater Manchester, we recognize the priority to achieve a parity between mental and physical health care and are committed to achieving this, enabled by our CORE 24 Liaison Mental Health services.

Our services work at the interface between physical and mental health to bridge this gap.

Our teams operate 24/7 to provide responsive mental health care in the acute hospital and A&E settings, delivered by experienced and established multidisciplinary teams.


Trafford Home Based Treatment Team are a committed integrated service that strives to provide quality recovery based support to people with severe and enduring mental health related difficulties in Trafford 7 days per week, 24 hour service.

The service calls upon a wide range of evidence based holistic assessments and interventions. You will be expected to complete home visits to support people in crisis and help to avoid hospital admissions.

You will be required to liaise with ward staff to help facilitate early discharges in a proactive and timely way.


There is a requirement to provide clinical leadership and to assist in the organising, supporting, supervision and development of junior staff members.

You will also be expected to act as assessor/mentor for student nurses. The post holder will play a key role in the further development of the service and in the implementation of its Clinical and Social Care Governance agenda
