Job Overview

We are seeking a highly skilled and motivated individual to join our team as a Trainee/Advanced Nurse Practitioner. This is an exciting opportunity to develop your skills and expertise in a dynamic and supportive environment.

Main Responsibilities
  • Provide advanced and expert level of practice within the Home Based Treatment (HBT) setting, leading to improved patient outcomes.
  • Support the clinical and operational team to deliver on service improvements in the quality of care and safety through service development, evaluation, and audit.
  • Develop and lead nurse-led clinics, providing assessment, formulation, and brief interventions of care, including discharge planning within agreed protocols and procedures.
  • Hold a caseload and lead on practice and service developments, demonstrating clinical and practice expertise via interventions, supervision, and modelling good practice.
  • Contribute to a HBT education and training programme across the community teams within Trafford.
  • Participate in the BOC/BOS on-call rota.
