We are seeking a highly skilled and experienced Community Psychiatric Nurse to join our team as a Care Coordinator. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality care to individuals with severe and enduring mental health-related difficulties.
Key Responsibilities- Undertake the role of Care Coordinator in line with the Care Programme Approach (CPA) process, working with a defined complex caseload of individuals who are on the Assertive Outreach pathway.
- Provide clinical leadership and support to junior staff members, as well as act as an assessor/mentor for student nurses.
- Contribute to the further development of the service and the implementation of its Clinical and Social Care Governance agenda.
- Registered Mental Health Nurse with at least 1 year post-registration experience.
- Comprehensive knowledge of CPA, Mental Health Act, and Capacity Act.
- Experience of mentoring and/or supervision, as well as working in the community, is desirable.
