£24,960 annually, with additional payments for shared on-call service up to £2,520 per year.

Hours: 40hrs per week + shared on-call service

We are looking for a Care Coordinator to join our team in Swindon, supporting our growing workforce.

Main Responsibilities:
- Assign care workers to customer visits through efficient rostering
- Maintain continuity of care and consider specific requirements
- Regular communication with care workers, offering guidance and resolving issues promptly
- Collaboration with other healthcare professionals
- Ensure all documentation is completed and updated in relevant systems
- Handle confidential matters with integrity
- Approach clients with empathy and understanding, respecting their individuality

Essential Requirements:
- Proficiency in using computers and office tools
- Demonstrate honesty, empathy, non-judgement, kindness, and respect
- Strong organizational skills and ability to work well under pressure

Desired Qualifications:
- Health and social care certifications (NVQ 2/3 or higher)
- Valid driver's license and access to a vehicle
- Previous experience in diverse healthcare environments
