Job overview

An exciting opportunity has arisen for a highly motivated, caring and enthusiastic practitioner with a minimum of 3 years' experience at band 6 as an adult nurse, occupational therapist or a mental health nurse to join our dedicated Dementia and Delirium (DAD) Team. The post holder will have previous hands on experience of caring for patients with dementia and/ or delirium and supporting their carers. You will also feel confident delivering DAD related training. The post holder will also have a sound working knowledge of and experience of using objective cognitive assessements either in an inpatient or outpatient setting (such as Memory Clinic).

Main duties of the job

Job Summary :

To work in Practitioner-led Memory Clinic (outpatient setting) To work as part of the DAD specialist practitioner team across the inpatient setting to effectively co-ordinate and deliver an expert DAD service To work with the DAD nurses to deliver education across the trust and across the multi-professional team To ensure that patients and their families are fully informed and supported by providing excellent clinical and social support To work within established clear lines of communication with primary healthcare teams in order to increase knowledge and facilitate the care of this group of people in the community.
