Job summary

We are looking for an enthusiastic individual with a commitment to psychological care to work with the cancer psychological therapies team in the role of a Support Worker.

This position is currently based at the Royal Sussex County. The successful candidate will provide support, care and assessment of patients and immediate family members accessing this service. This will involve a combination of administrative work, and patient contact, primarily by phone and email with some face to face and online work . The post holder is often the first point of contact for patients and carers and will require excellent communication skills, be highly organised and be able to work independently and also work alongside other members of the Psychological therapies team and liaise with other staff within cancer services and in primary care. Experience of working within the NHS is an advantage and being comfortable with NHS policies & procedures is necessary.

This is a permanent position working Monday to Friday.

Main duties of the job

The Psychological therapies Support Worker will work alongside and under the guidance of Macmillan Lead for psychological therapies & the cancer services psychological therapist to provide support, advice and assessment for patients who have been diagnosed with cancer and referred to the psychological therapies team. Often being the first point of contact the post holder will triage calls and offer advice and support as able signposting and referring on as required.

They will take an active role in contributing towards the running and development of health and wellbeing events and support the psychological therapists in achieving Macmillan Quality Standards.

The post holder will provide a link between the cancer service psychological therapists and other professionals ensuring referrals are organised and prioritised according to need.

Job responsibilities

Assess appropriateness of referrals and signpost on as necessary Triage calls and offer support and advice. Ensure all referrals have had a HNA completed before processing the referral further. Assist and help develop and co-ordinate education and support events Support people to access appropriate information and support which promotes self- management Advise on healthy lifestyle post treatment and assist with accessing services which helps independent living with a cancer diagnosis Guide patients through their cancer pathway co-ordinating appointments as required Co-ordinate & organise appointments and advise on assessments as required. Support patients calling for support who may be waiting anxiously for their appointment Triage contacts from patients, relatives and staff prioritising the designated workload in relation to identified need Contribute to the holistic needs assessment and the development of care plans Co-ordinate and organise appointments, assessments and referrals as required Co-ordinate the handover of care with other teams to facilitate safe and effective transition of care between services in order to provide seamless support Make pre-planned outbound phone calls to offer support and assess ongoing needs to enable a proactive prevention approach Identify need and changes in management required through telephone contact and respond appropriately, including rapid re-entry into the system for those identified as having urgent or specialist needs

oGCSE English Language and Mathematics (Grades A-C) or equivalent oNVQ4 or training in foundation degree in health and social care equivalent, or equivalent through short courses
