We are seeking a highly skilled and compassionate Mental Health Practitioner to join our team at Sussex Partnership NHS Foundation Trust.
As a key member of our primary care service, you will work closely with clients, carers, and other professionals to provide high-quality, evidence-based psychological therapies.
Key Responsibilities- Assess and support individuals with common mental health problems, promoting self-management and recovery.
- Conduct patient-centred interviews to identify areas for change and risk assessments.
- Make decisions on suitability for therapy and clinical pathways, referring unsuitable clients to relevant services as needed.
- Provide a range of information and support for evidence-based high-volume low-intensity psychological treatments.
- Collaborate with other professionals to ensure seamless care and effective communication.
- Qualification from a PWP Training Course or equivalent experience as a Graduate Mental Health Worker.
- Experience working with people who have experienced mental health problems.
- Excellent written communication skills and ability to write clear reports and letters.
- Experience in Primary Care Services.
