Working Full time : 37.5 hours per week

We are excited to offer an opportunity for a B4 Associate Practitioner to join the Discharge Lounge team at the Countess of Chester Hospitals NHS Foundation Trust.

The aim of the discharge and transport lounge is to promote patient flow, improve quality of care and patient experience for patients being discharged from the Countess of Chester.

The post holder will be required to liaise with patients, families, carers, ward staff, internal departments and external transport agencies to ensure that patients being discharged via the hub experience a coordinated journey from ward to hub to home.

Main duties of the job

The post holder will be an accountable, professional, responsible member of the team and will deliver high quality, patient centered, compassionate care to patients being discharged via the discharge and transport hub. The post holder will assume responsibility and accountability for the team within the discharge lounge and provide a safe caring environment that promotes dignity, privacy and respect.

To coordinate tasks to ensure patients are discharged safely and timely from hospital.

To provide support to the multi professional team who work within the discharge lounge.
