This is a supportive role to the Lead Health and Wellbeing Coach for Young People.

The role will be varied and include offering holistic wellbeing assessment of young people aged 0 to 19 through our single point of access clinic based at a Wisbech GP practice.

Gathering information to understand how best the young person and family can access support; this role will entail referring internally to our SEEN team as well as externally to specialist services such as secondary care/ community services.

Working with a multidisciplinary team to advocate for young people is paramount, gathering information and assessing the most appropriate route for support and/or intervention.


The assessment will be based on the biopsychosocial model, exploring holistically the factors that influence wellbeing, such as; physical, emotional, intellectual, vocational, economic, social, spiritual, and environmental factors.

In addition, the post holder will provide coaching support to young people aged 12 to 19 working in partnership with clinical and non-clinical professionals, social care and voluntary sector colleagues to ensure the best possible outcomes for the young person.

Also offering support to our team to facilitate well-being drop-ins and peer support groups.

Main duties of the job Take referrals from the SEEN single point of access when it has been identified that patients meet the criteria for health and wellbeing coaches Work in partnership with clinical and non-clinical colleagues, to explore all contributing factors to wellbeing.

Support patients to discuss goals, progress and difficulties. This may be face-to-face or by telephone Creating secure, supportive environments for patients to explore their objectives and intentions Assist patients in gaining a better understanding of their reasons and changing behaviours Assist patients in building health plans that are tailored to their specific needs Positively communicate and negotiate to support patient personal choices Support patients to create health objectives for themselves Understand when it is appropriate or necessary to refer people back to health professionals when the patients needs are beyond your scope Manage and prioritise your own caseload in accordance with the needs, priorities and urgent support required by your patients Be a friendly source of information about wellbeing and prevention approaches by supporting the SEEN team to develop and delivery health and wellbeing seminars for family members, patients and even employees who are interested in learning more
