This is an exciting opportunity to work as part of a multi-disciplinary team in the South Bristol Locality with a focus in delivering urgent care.

You will be required to respond to urgent referrals in need of complex management in order to prevent hospital admissions and facilitate early discharges.

You will have continuing responsibility for proactively monitoring and managing patients on the caseload to ensure the best outcomes.


There will be an opportunity to develop your skills in physical assessment and clinical reasoning with the support of Advanced Clinical Practitioners and wider members of the team.

There is also the opportunity to support the REACT service in preventing admissions within the Emergency Departments at the two main Acute Trusts in Bristol (UHBW and NBT).
