About the Role

We are seeking an experienced and motivated Paramedic or Advanced Nurse Practitioner to join our multidisciplinary clinical team at Armada Family Practice. As an Autonomous Healthcare Professional, you will play a key role in delivering high-quality patient care in a primary care setting.

Key Responsibilities
  • Provide holistic healthcare for a wide range of health needs, including undifferentiated and undiagnosed problems, using advanced clinical skills.
  • Act autonomously within your professional boundaries to provide clinical care to patients.
  • Complete a weekly ward round for each care home to assess new residents and acute/chronic problems.
  • Be the main practice contact for care homes and ensure completion of patient care plans.
  • Ensure completion of the PCN DES requirements and provide information needed to assist with medication reviews.
  • Demonstrate safe, clinical decision making by providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care.
  • Maintain knowledge of medical developments in primary care and ensure all new clinical guidelines are adhered to.
  • Support the practice with regular audits as required by NHS England and Bristol ICB for identified key practice developments, particularly in relation to long-term condition management and QOF aims.
  • Work collaboratively and with the multi-disciplinary practice team to meet the needs of patients.
Requirements
  • Experience in triage/initial assessment both face-to-face and over the telephone.
  • Experience of managing patients with minor ailments.
  • Experience of managing elderly/frailty in primary care.
  • Can demonstrate competency in autonomous practice.
  • Evidence of up-to-date knowledge of national and local policies, including relevant NICE guidelines.
  • Understanding of evidence-based practice.
  • Can demonstrate examination skills.
  • Knowledge of adult safeguarding.
  • Evidence of personal professional development.
Desirable Qualifications and Skills
  • Management of patients with complex needs.
  • Understanding of clinical governance issues.
  • Understanding of scope of professional practice and its implications for advanced practice, including personal accountability.
  • Excellent communication skills with a variety of people and using a variety of forms (written, verbal).
  • Time management and the ability to prioritize your own workload.
  • IT skills.
  • Ability to develop effective working relationships within a multidisciplinary team.
  • Access to a car/transport.
