Position Overview

Armada Family Practice is pleased to announce an opportunity for a dedicated and proactive Advanced Nurse Practitioner or Paramedic to become a vital part of our collaborative and welcoming healthcare team, working 16 hours per week.

We are in search of a qualified Paramedic or Advanced Nurse Practitioner with a keen interest in geriatric care to join our diverse clinical team. The ideal candidate will possess the ability to work independently and effectively to perform patient evaluations, diagnoses, and management of health conditions within a residential care environment.

Key Responsibilities

In this role, you will collaborate with our Partners and Salaried General Practitioners to provide exceptional patient care as an Advanced Practitioner in a primary care setting. Utilizing advanced clinical skills and comprehensive theoretical knowledge, you will be equipped to assess and manage both acute and chronic health issues in a care home context. Your responsibilities will include:

  • Conducting weekly visits to each care home, evaluating new residents and addressing acute and chronic health concerns.
  • Engaging with patients and their families as necessary, ensuring that care plans are completed and communicated effectively.
  • Supporting the practice team in maintaining high standards of patient care.
  • Collaborating with multidisciplinary teams to ensure comprehensive care delivery.
  • Keeping abreast of medical advancements in primary care and adhering to new clinical guidelines.
  • Participating in regular audits as required by healthcare authorities, particularly in relation to long-term condition management.

Armada Family Practice operates from a modern, purpose-built facility, serving a patient population of approximately 16,300 individuals. Candidate Profile

Essential Qualifications and Experience

  • Registered with NMC/HCPC
  • Independent prescribing qualification
  • MSc in Advanced Clinical Practice
  • Experience in triage and initial assessments, both in-person and via telephone
  • Proven ability to manage patients with minor ailments and those with complex needs
  • Knowledge of adult safeguarding and evidence of ongoing professional development

  • Familiarity with the practice clinical system EMIS
  • Strong communication skills, both written and verbal
  • Effective time management and prioritization abilities
  • Access to transportation
