Job summary

We are looking for an enthusiastic 7 Pharmacist who is passionate about the care of frail, older people to join the Haringey (West) - Enhanced Health in Care Homes Team.

The national EHCH model and its requirements are a multi-disciplinary and multi-facetted approach which is outlined in a National EHCH framework . It's implementation relies on close collaboration between partners (including care homes themselves), particularly primary care and community healthcare services. It is part of progressing the NHS Long Term Plan goal of "dissolving the historic divide" between the two sectors and sets a minimum standard for NHS support to people living in care homes.

This is an opportunity to be part of the newly formed Haringey (West) EHCH Team as part of the wider Haringey Proactive Care /Multi-Agency Care and Coordination (MACC) Team.

The Haringey (West) EHCH is a nurse led service. As part of the wider MDT you will also be actively working alongside and Trusted Assessors, care home staff and GP Practices. The MACC Team managers will support from an operational perspective.

The Haringey (West) EHCH will focus on intervention in care homes, providing specialist support for residents, resident's family, their carers and GPs.

Main duties of the job

Working in six Haringey (West) care homes, the Pharmacy role will be to support with medication advice, input and review as part of the initial assessment as well as planned and unplanned reviews. The Pharmacist will be working alongside the consultant geriatricians and the wider multidisciplinary MDT in order to carry out Structured Medication Reviews. You will also be part of the monthly multidisciplinary meetings for each care home which allow for better development of trust between the MDT members, better integrated working with Care Home staff and GPs improving the patient/resident experience. The Pharmacist will carry out comprehensive medication reviews for frail patients both in their own home (residential placement) or over the phone where this is appropriate. These patients often have polypharmacy and multiple complex comorbidities.

Job responsibilities

The Pharmacist will support medication reviews for those who are referred directly to the service or those who meet the referral criteria face to face at the residential homes.

The structured medication reviews are conducted in line with the standards of medicine optimisation, to ensure the safe and effective use of medication in collaboration with the wider multidisciplinary team to provide holistic medication reviews.

A medication review will involve the completion of an SMR tailored to the needs of the resident with their involvement. The EHCH Pharmacist will proactively promote safe and evidence-based prescribing with the nominated general practitioners.

The EHCH Pharmacist will be involved in de-prescribing where required, ensuring safe and effective reduction of stopping inappropriate medicines.

The Pharmacist will support local care partners to provide care to elderly patients in community and engage in new projects. The pharmacist will participate in audits, to implement cost-effectiveness.

To work in collaboration with the GPs, consultant geriatricians and the wider multidisciplinary team to provide holistic medication reviews.

To raise the profile of care home pharmacists and engage in new projects with the North Central London ICB to improve medication safety and practices in care homes.

To support local care partners (such as GP federations, Primary Care Networks and community pharmacy colleagues) provide care to elderly patients in the care homes, if required.

Demonstration of an understanding of the specific clinical aspects of medicines management, in this clinical frailty speciality.

To support implementation of national guidance in medicines management for older people in the care homes with complex pharmaceutical care needs.

To act as a mentor and role model to junior staff within Pharmacy.

Education/ Qualifications

MPharm (or equivalent) Registered with the General Pharmaceutical Council (GPhC) Completion of a certificate in Pharmacy Practice/ Clinical Pharmacy, or equivalent
