About the Role

We are seeking a highly skilled and experienced Pharmacist to join our Enhanced Health in Care Homes (EHCH) Team at Whittington Hospital NHS Trust. The successful candidate will play a crucial role in supporting the care of frail, older people in care homes across Haringey.

Key Responsibilities
  • Conduct comprehensive medication reviews for frail patients in care homes, ensuring safe and effective use of medication.
  • Work collaboratively with the multidisciplinary team, including consultant geriatricians, GPs, and care home staff, to provide holistic medication reviews.
  • Support the implementation of national guidance on medicines management for older people in care homes with complex pharmaceutical care needs.
  • Act as a mentor and role model to junior staff within Pharmacy.
Requirements
  • MPharm (or equivalent) degree.
  • Registration with the General Pharmaceutical Council (GPhC).
  • Completion of a certificate in Pharmacy Practice/Clinical Pharmacy, or equivalent.
  • Good interpersonal and communication skills, written and oral.
  • Able to communicate with patients and all grades of healthcare staff (verbally and in writing).
  • Able to influence and inform decisions about the pharmaceutical management of patients.
  • Appropriate post-qualification experience in the acute NHS sector.
  • Demonstrates awareness of and commitment to the principles of clinical governance.
  • Experience of multidisciplinary team working.
