Job summary

An exciting opportunity has arisen for a highly motivated individual to join our Hepatology team as a Pathway Navigator. The Hepatology Pathway Navigator will support the Liver Team in proactively tracking patients with liver cirrhosis along the diagnostic pathway, including discussion at the weekly HPB MDT meeting for any suspected liver cancer patients. They will ensure that accurate data is collected to maintain precise patient pathway information on the Liver Tracker for regional data reporting. They will also support the Trust in achieving National accreditation for our local liver services.

We are looking for an individual who is passionate about delivering earlier cancer pathways by ensuring a coordinated and smooth service for patients through their pathway.

We are keen to hear from individuals who have the necessary drive and motivation to support service delivery.

Main duties of the job

The Hepatology Pathway Navigator role will be involved throughout the patient's pathway, including supporting the Hepatology team in identifying patients requiring hepatocellular carcinoma (HCC) surveillance and acting quickly on results to ensure that all patients move through the pathway without delay which will ultimately lead to a quicker diagnosis and treatment.

Job responsibilities

With frequent contact with patients and their families, it is essential that the post holders are emotionally resilient with a caring and sensitive attitude. They will also have excellent communication skills to liaise with a wide range of health professionals to ensure tests and appointments are coordinated and scheduled in a timely manner and patients are kept informed at all times.

The ideal candidate will have an understanding of cancer targets and patient pathways. It is desirable that they have previous NHS experience in a patient facing role, a good level of education, experience of using Cancer Waiting Times systems, clinical IT systems and are competent in Microsoft Office packages. We are looking for enthusiastic and conscientious team players with excellent time-management and organisational skills who can work autonomously and well under pressure.

Person Specification

Education & Qualifications

Essential

Maths and English GCSE's at grade C or equivalent. Evidence of continued role development and willingness to learn and develop. NVQ 3 in related subject or equivalent experience.

Knowledge

Essential

Computer literate with good knowledge and understanding of computer packages and databases, Microsoft Office, Excel. Awareness of patient confidentiality within the NHS.

Desirable

Knowledge and experience of using Trust IT/patient record/information systems, Medway, CRIS, CWP. Knowledge of medical terminology.

Experience

Essential

Previous experience of working with the public and/or patients in a healthcare environment. Previous administrative and organisational experience.

Desirable

Experience in Gastroenterology services.

Skills & Abilities

Essential

Ability to work without direct supervision. Good organisational and administrative skills with ability to prioritise workload and exhibit high level of concentration.
