Main duties of the job

Actively support the organisation and share responsibility for the provision of a specialist COPD Service to facilitate the achievement of COPD objectives within the local and national agendas in COPD management.

Perform strategic role as nurse lead in the on-going improvement, delivery, and evaluation of COPD services within Stockport, in particular, focusing on the development and provision of community based services for the management of COPD.

Work collaboratively with colleagues and managers to develop partnerships and enhanced care pathways within the Integration of health and social care programme.

Work as a specialist autonomous practitioner, effectively coordinating the delivery of an expert service within COPD. Supporting health and social care key professionals, in accordance with local and national guidance, in order to promote guideline compliance. Also acting as an expert resource and support to patients and relatives/ carers. Line manage staff within the COPD, Oxygen, Respiratory Medicine and Pulmonary/Heart Failure Rehabilitation services, offering leadership, support and guidance.

Job description

Job responsibilities

Act as a specialist resource for key partners within Stockport Neighbourhood Care

Promote the integration agenda within Stockport Neighbourhood care and ensure that all patient care is holistic and in keeping with the vision of Enhanced case management.

Participate within MDT triage meetings within the Neighbourhoods Specialist Clinical Lead for Stockport Foundation Trust COPD Service, coordinating expert, evidence-based respiratory disease management and care for patients with COPD.

Supervise the provision and delivery of COPD services by the team, working in collaboration with managers, nursing, medical and allied healthcare professionals across primary and secondary care to improve COPD expertise and care.

Develop and implement nurse/AHP-led clinics for COPD management in primary care settings, using advanced specialist skills and knowledge to ensure consistent high standards of care.

Critically evaluate and challenge current practice and processes of COPD care and management, identifying and initiating appropriate steps to assist with innovative solutions to modernise and develop services to achieve local, Trust and National objectives.

Plan, implement, monitor and review individualised patient care, ensuring optimal treatment is achieved, and quality of life maintained/ enhanced, advising other clinicians and GPs accordingly to improve outcomes for COPD patients across primary and secondary care.

Provide enhanced specialist advice, education and support to patients with COPD, their families/ carers and other healthcare professionals.

Lead on the identification, development and evaluation of clinical protocols and strategies to enhance both continuity and standards of specialist COPD care whilst ensuring equity of access into the service (clinic and domiciliary) via numerous pathways/ referral routes.

Work collaboratively in relation to clinical guidelines, training, research, audit and evidence-based practice Identify actual or potential clinical risks, taking appropriate action, using when required the appropriate reporting structures, in accordance with Trust policies and guidelines.

Work across organisational boundaries and in conjunction with stakeholders, managers, clinical and non-clinical staff, multidisciplinary teams and agencies to promote COPD pathways and services.

Accountable for own professional actions and clinical decisions, acting as an advanced independent practitioner/ decision maker.
