We are seeking a highly skilled and experienced Respiratory Nurse Specialist to join our team at Stockport NHS Foundation Trust. As a key member of our community COPD service, you will play a vital role in delivering high-quality, patient-centered care to individuals with chronic obstructive pulmonary disease (COPD).
Key Responsibilities- Assess and manage patients with COPD in the community, including those with complex needs and those requiring oxygen therapy.
- Develop and implement individualized care plans to optimize patient outcomes and improve quality of life.
- Work collaboratively with multidisciplinary teams, including respiratory consultants, GPs, and other healthcare professionals, to ensure seamless care coordination.
- Provide education and support to patients and their families to promote self-management and independence.
- Contribute to the development and implementation of clinical guidelines and protocols to improve COPD care.
- Participate in quality improvement initiatives to enhance patient outcomes and service delivery.
- Current registration with the Nursing and Midwifery Council (NMC).
- Postgraduate qualification in respiratory nursing or a related field.
- Experience in community COPD care, including assessment, management, and education.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work autonomously and as part of a team, with a flexible approach to meet the needs of the service.
- Knowledge of COPD pathophysiology, assessment, and management, including oxygen therapy and non-invasive ventilation.
