About the Role
The role involves providing tailored support to house rough sleepers, aiding them in developing independent living skills and general well-being. The goal is to help individuals with a history of rough sleeping achieve independence and transition away from rough sleeping.
A Day in the Life
As a Tenancy Sustainment Coordinator at Hestia, you will play a vital role in our Mental Health and Complex Needs Service, directly impacting the lives of our service users. Your tasks may include:
- Providing customized support to individuals previously or currently at risk of rough sleeping.
- Collaborating with referring agents at the H&F PATHS Team.
- Developing and maintaining independent living skills and general well-being.
- Creating personalized SMART support plans to help individuals achieve their goals.
- Engaging service users in activities that promote well-being and independence.
