Job Description

  • The post holder will be required to provide a high-quality resource of professional advice, guidance and expertise to members of the individual care team, ICB and wider health and social care economy.
  • Autonomously manage a case load of individuals with highly complex health needs providing significant clinical guidance to providers.
  • Ensure that all assessments are holistic in nature and apply a person centred approach.
  • To have an expert level of knowledge regarding management of complex care and to be able to impart this knowledge to a variety of audiences.
  • To provide complex and sensitive information to patients, relatives/ representatives and often work in potentially hostile environments, relaying difficult decisions regarding care provision and managing patient and family expectations.
  • Attend all relevant meetings related to the patient which may impact on the care provision and costs to meet identified needs.

The Complex Care Case Management Service provides a case management function for the population of BOB ICB. The post holder will-Manage a complex caseload. Work alongside MDT including, ICB, local authorities, acute trusts and legal team. Support with coordinating of day-to-day tasks and referrals. Supervise and mentor staff. Have opportunities to develop skills in a variety of areas including safeguarding, personal health budgets, clinical supervision, deprivation of liberty, mental capacity, and clinical nurse assessments for end-of-life pathways.
